What Makes Primary Care at Tone & Core Different
Primary care at Tone & Core is delivered with intention—grounded in time, continuity, and clinical judgment.
Care is offered through a cash-based monthly membership, rather than traditional insurance billing, with no co-pays, deductibles, or hidden fees for covered primary care services.
As a physician-led family medicine practice, we focus on understanding the whole person rather than addressing isolated symptoms, allowing care decisions to be informed, individualized, and transparent.
Appointments are relationship-driven, creating the space needed to listen carefully, evaluate thoroughly, and develop care plans aligned with each patient’s health goals, medical history, and lifestyle.
Our approach emphasizes:
Prevention • Clarity • Continuity of care • Long-term physician–patient partnership

What to Expect When You Walk In
Step 1 — Assess
Your visit begins with a comprehensive conversation about your medical history, lifestyle habits, symptoms, and health concerns.
This allows Dr. Vargas to understand the full context of your health rather than focusing only on isolated symptoms.
Step 2 — Evaluate
Vital signs are reviewed and a focused physical exam is performed to evaluate your current health status.
Evidence-based screenings may be recommended depending on your needs.
Step 3 — Test
When appropriate, personalized laboratory testing is ordered through our Lab partnerships to provide deeper insight into your health needs.
Step 4 — Plan
You leave your visit with a clear, customized care plan designed around your health goals.
This may include treatment recommendations, lifestyle adjustments, medication management, or preventive strategies.
Step 5 — Follow Through
Your care continues beyond the visit.
Patients have access to follow-up guidance, lab reviews, and ongoing support as their health evolves.
Appointments remain calm, unhurried, and focused entirely on your needs.

Frequently Asked Questions
Is the Tone & Core Primary Care model ideal for me?
This model is ideal for patients who value longer visits, direct physician access, personalized care, and a transparent cash-based structure without insurance limitations.
Is this a concierge medicine practice?
Tone & Core operates on a membership-based primary care model similar to concierge medicine, prioritizing time, continuity, and physician-led decision-making.
Do you take health insurance?
Primary care services are not billed to insurance. Care is provided through a transparent monthly membership. Many patients maintain insurance for hospital care, specialists, imaging, or emergencies.
Do I need health insurance under this primary care model?
While health insurance is not required to receive care at Tone & Core, we strongly recommend maintaining some form of health insurance. Many patients keep a High Deductible or Catastrophic Health Plan for non-primary care needs (e.g., emergency care, hospitalizations, surgeries, specialists, advanced imaging).
Can I still use my insurance for labs or medications?
Yes—laboratory testing and prescriptions sent to the pharmacy can typically be processed through your insurance if you choose. We also offer reduced-price cash-pay options through our Lab partnerships and in-house dispensing.
Do you see men, women, and teens?
Yes—primary care services are available for men and women ages 18 and older.
Can I get a same-day appointment if I’m sick?
Same-day or next-day visits are often available for acute illness, depending on availability.
Do you serve Sarasota or Bradenton?
Yes—patients visit us from Lakewood Ranch, Bradenton, Sarasota, and surrounding communities.
Do you prescribe controlled substances?
We do not prescribe the routine use of benzodiazepines or narcotic medications, nor provide chronic pain management. Our focus is on preventive care, wellness, and addressing health concerns through evidence-based and individualized care whenever possible. On occasion, controlled medications may be prescribed when medically appropriate and in accordance with Florida regulations and DEA guidelines. When appropriate, we will help coordinate care with specialists equipped to manage these needs.
I have Medicare. Can I still join?
Yes—patients with Medicare can join the membership program by signing a “Private Contract” as required by law. This acknowledges that the practice has opted out of Medicare, and neither you nor the provider will submit claims to Medicare or seek reimbursement for services covered under the membership.
How this affects your coverage:
- Primary Care: Your membership fees are paid out-of-pocket and are generally not covered or reimbursed by Medicare or Medicare Advantage (HMO/PPO) plans.
- Outside Services: You can still use your Medicare benefits for services not performed in this office, such as specialist visits, hospitalizations, lab work, and imaging.
- Plan Restrictions: If you have an HMO, your insurance may still require you to have a designated “in-network” PCP on file to authorize or cover those outside specialist visits and tests.
For specific questions regarding your plan’s “out-of-network” rules, please contact your insurance carrier directly.
Is my doctor available 24/7?
Members have direct physician access for follow-up questions and care coordination during appropriate hours. For emergencies, patients are directed to appropriate urgent or emergency care.
What happens if I need to go to the hospital or see a specialist?
Dr. Vargas will help coordinate referrals and ensure continuity of care, communicating directly with specialists when needed.
Is Xeomin® different for men vs. women?
Dosing and placement are often adjusted for men due to stronger facial muscles and different aesthetic goals (e.g., avoiding over-softening). Our physician-led approach ensures results enhance masculine features naturally.
What happens if my doctor is out of town or unavailable?
We make every effort to ensure continuity of care. Patients receive clear guidance on how to obtain appropriate care if Dr. Vargas is temporarily unavailable. For urgent or emergent medical issues, patients should seek care through urgent care services or the emergency department as appropriate.
Is this a good option for small business owners and their employees?
Yes—many small business owners and employees appreciate the predictable cost, direct access to care, and focus on prevention.
How do I pay for my membership fee?
Membership fees are typically paid through secure recurring monthly billing.
Is there an enrollment fee to join?
Yes. A one-time, non-refundable enrollment fee of $100 is required at enrollment to cover administrative setup and onboarding.
Can I cancel my membership?
Yes—you may cancel the membership at any time. Cancellation terms are outlined during enrollment.
