Enrollment Fee
A one-time, non-refundable $100 enrollment fee applies to new memberships.
Monthly Billing
Membership fees are billed monthly.
Cancellation
Membership cancellation requires 30 days' written notice.
Limited Enrollment
Primary Care membership is intentionally limited to preserve physician access, adequate visit time, and the personalized nature of care.
Re-enrollment
Patients who discontinue membership and later wish to return are subject to membership availability and a $500 re-enrollment fee.
Full Transparency
What's Not Included
Membership fees cover physician care and program management as described above.
Prescription and compounded medications, laboratory testing, imaging, supplements, injections, medical devices, aesthetic and regenerative treatments, and services provided by outside facilities or specialists are not included in the monthly membership fee unless specifically stated otherwise.
All prescriptions, testing, treatments, and participation in Optimization programs are based on individual medical needs and clinical appropriateness.
We are a direct-pay practice and do not bill health insurance for membership services. You may continue to use your health insurance independently for medications, laboratory testing, imaging, specialist care, hospital services, and other outside services when applicable.
